Register for our online service

To register for Online Services, please complete the relevant Online Access form (according to your age), along with Identity Documents Disclaimer.

Two forms of identification are required with the registration form, either with electronic copies or at Reception.

  • One ID item must include your photograph
  • One must contain your address and one must contain your signature.

The ID item confirming the address must be dated/valid within last 3 months.  Any items emailed to the practice must be accompanied by an electronic copy of a signed and dated Thurleigh Road Practice Identity Document Disclaimer which is available here –

You will then receive your own unique username and password.

One you are registered you will be able to use the service to:

  • Order repeat prescriptions
  • Make appointments
  • Cancel an appointment
  • Update your contact details
  • Review your medication and known allergies
